(Spanish) Yamhill County Public Health invites community members to attend a COVID-19 vaccine clinic at Duniway Middle School on Wednesday, May 19 between 3 – 7 p.m. The event will take place in the covered area near the front of the school.

Community members ages 12 and older are eligible. Walk-ins will be accepted as time allows.

Note: students ages 12 – 14 must have signed parental consent to receive a vaccine. Download the consent form here. The clinic’s provider, Providence, has also indicated that consent can be a written note from a parent or guardian. Click to read the fact sheet on the Emergency Use Authorization of the Pfizer vaccine.

Parents do not need to attend the event if students have the signed form.

Please bring a photo ID and a health insurance card (if you have one) with you to the event. Insurance is NOT required to receive a COVID-19 vaccine, and the vaccine is available at no cost to you.

If you need assistance signing up for an appointment please email Yamhill County Public Health at covid@co.yamhill.or.us or call 503.474.4100.
