Students to be asked for input to gauge statewide resources and support

The Oregon Department of Education (ODE) is looking for input from students about their school experiences. The Student Educational Equity Development Survey (SEED) will help ODE develop appropriate resources and supports for Oregon districts and better target those resources and supports where they are most needed. 

The SEED Survey gathers data from students statewide, in grades 3-11. The content of the survey varies by grade level.  

Students in grades 3-11 will be asked to take a 10–15 minute survey at school sometime between May 5 and June 7, 2024

The survey will ask students:

  • About the kinds of activities they did in school this year;
  • What supplies did they had to help with schoolwork,  and
  • How they felt about school and learning this year.
  • Additional Helpful Information

    • You may decline participation of your child for any reason. (Opt-out forms for English or Spanish. If you need the form in another language, please send message to info@msd.k12.or.us)
    • Students can skip questions within the survey if they do not want to or cannot respond for any reason.
    • The survey will be available for students experiencing significant cognitive disabilities that captures the same information with the support of a teacher or staff member.
