How to Request a Transcript

If you attended or graduated MORE THAN three years ago, please click for the transcript request form (Spanish). If you have questions or need help with the request from, please contact McMinnville School District Office at 503.565.4004.
Information you will need:

The first and last name you used as a student. This should be your legal name.

Your date of birth
-
Your telephone number and/or email address for questions

Your current mailing address

If not sent to you, the mailing address, email address, or fax number you want the transcript(s) sent

Number of transcripts sent and whether they are official transcripts or unofficial transcripts

Do you want your SAT and/or ACT scores included in the transcript? They will not be included unless you specifically request them.

Remit $3 for each official copy by cash, check, or money order to: McMinnville School District / Transcripts, 800 NE Lafayette Ave, McMinnville, OR 97128
